This application is used by pain management practitioners to assess patient's disability levels using a standard set of pain assessment questionnaires.

Workflow consist of the following steps:

- The practitioner selects questionnaires to be filled out by the patient.
- The patient selects appropriate responses to all the questions.
- Detailed assessment in PDF format is emailed to the practitioner.
- The practitioner has an option to also print this report
